    Please fill out the information below and hit the "submit" button when complete. We will use your chosen form of contact to reach out and schedule an appointment according to your preferences. Please keep in mind this is a request, and an appointment is not scheduled until we confirm a date/time by email, call or text with you. We try our best to respond within 3 business days, but after holidays it may take a little longer.
    (Saturday Clinic Appts. Will Be Scheduled between 8am - Noon.)
    Tech Only Preventative Care - Vaccinations, Heartworm Prevention, Heartworm Testing
    Veterinary Exam - Sick, injured, checkup. A $40 prepay is required (you can make this payment at https://square.link/u/6V9gy8yF or with a pets representative when you are contacted with the appointment date/time). The $40 is refundable, as long as we get at least 24 hour notice for cancellation.
  • For any services requiring a veterinarian (all but preventative tech only) we will need to reach out to confirm your appointment time and collect a prepayment, please make sure we can reach you on this number.
  • By typing my name above and submitting this form, I hereby agree that the procedures agreed upon in the followup call are approved by me. To my knowledge my cat/dog is healthy and has not bitten anyone in the past ten days. I agree to hold harmless and release from liability P.E.T.S. Low Cost Spay and Neuter Clinic from any illness or fatality that results from my pet receiving treatment(s).
